Do you put todos in your notes?

I've found personally, as well as observing others - todos just happen to get captured over the course of taking notes. Typically this happens during a meeting or perhaps just generically taking notes on something. You're writing and boom, an action item comes up.

The problem is you end up having todos sprinkled all around and it becomes difficult to track OR you have to put todos elsewhere and they become decoupled from the source (the note).

Do others face this problem? If so, do you end finding your todos become lost/never acted on?
