I guess what works for me is having a clear and predictable routine for each day of the week.
I start off the day with meditation followed by a good breakfast and ease into daily TODO. This would include checking my emails, replying to my co-workers and laying out the required tasks for the day.
I also make it a point to get some sort of exercise each day in the evening. This gives me a good break after the day's work and before I start working on my personal TODO list.
I also avoid looking at social media post 8 pm and this actually has improved my focus a lot. Apart from that I also make it a point to read a book for 15 mins each day.
Everyone will have a different routine and it takes a few iterations before you realise what works best for you.

I start with setting up my day - what's important and needs full attention and which tasks I can do beside.
For the important tasks, I activate the do not disturb mode on my iPhone / Macbook. Social Media I don't use in general (only business related).
I don't want to be disturbed while working.
Beside that I drink a lot of water and try to get enough quality sleep every day. Try to go for a walk every day at evening.
What helps me a lot in the moment (YMMV) is using white noise to give me a predictable, uniform soundscape to avoid distracting sounds. Also helps to disable non-critical notifications to keep that state of flow going. Taking breaks is still important, though.
Apart from that, most of what helps me focus happens outside of work: get enough (quality) sleep and avoid ODing on sugar where possible. Mindfulness exercises are also helpful, especially on days where my mind is especially busy.
